- ObjectiveTo compare the effects of a high-velocity low-amplitude thrust (HVLAT) to the lumbar spine on pain pressure thresholds of the paravertebral and hamstring muscles. DesignTwenty-one asymptomatic subjects participated in a randomised, single blinded cross over design study. Subjects were randomly assigned to either; HVLAT group, sham ultrasound group, or a control group. All subjects were assigned to each intervention with a one-week wash out period. Pain pressure threshold (PPT) measurements were taken pre and post intervention, using an algometer on the right paravertebral muscle at levels L4/L5 and the right bicep femoris muscle. A two way repeated measures ANOVA was conducted to determine pain pressure threshold measures between groups, and gender. Cohen’s d and Pearson product moment correlation was conducted to measure effect size and the effects of age. Statistical significance was set at 0.05. ResultsStatistical significant increase was detected between treatment and time (LES p=0.001, hamstring p=0.001). Analysis revealed an increase in mean PPT values post intervention between HVLAT intervention versus sham (p=0.018) and control groups (p=0.014) at the LES with a medium effect size (d=0.69). No significant findings were detected at the hamstring between groups (p>0.05). ConclusionHVLAT to the lumbar spine caused a statistically significant increase in mean PPT values at both local and distal sites. Further investigation into the long-term benefits in conjunction with other treatment modalities, in symptomatic subjects is needed.
